In current and last organization I found some strange issue where the approval workflow set at library level works intermitently fine.
Here are my settings in Approval workflow:
enabled -Allow this workflow to be manually started by an authenticated user with Edit Item permissions.
enabled -Start this workflow when a new item is created.
