SharePoint 2013 List Permissions - Add Item ONLY

Hi all,

I have a "form" that is tied to a list that I created in Foundation 2013.  I do not have Design Manager, nor Infopath, so this is the only way to create the form.  I have my home page of the site set up with a direct link to the add item of the form, so that when the user clicks on "Request Access Form" they are taken directly to the add item page without even realizing it.  However, I do not want a user to be able to open this list and see information from other users.  I have changed the permissions to Add Item, but the View Item is also checked by default.  Is there a way to be able to change these permissions? 

Or will I need to create a workflow (I only have the three step) to copy the information from list A to list B and then delete the information from list A. Is this workflow even possible - I would need it to be automatic.  And then when the information is automatically added to list B I would need that list to fire off another workflow/email to the person responsible for processing the request.

Any help would be greatly appreciated. We use OOTB SharePoint Foundation 2013.
