Create a Work Plan List and work hours submission

Hi,

Im using sharepoint 2013 and i want to create a list the will contain a task name, phase of the task, employee assigned, and columns for monthly planned hours to this month and the next 3 months.

Its should be like that:

what current month = March 2013 , and we need that by the end of the month its will create a new column with the next month updated.
